Student names or grades (optional and only when required for event volunteer placement)

Forms are stored securely through our website platform and/or Google Workspace.
Only Life Link leadership and relevant LCS staff have access to submitted information.
Data is used only for Life Link–related purposes.

5. Children’s Privacy
Life Link does not knowingly collect personal information directly from children under 13.
All information collected about students is volunteered by parents or guardians for event participation or sign-up purposes.
